(Due to cancellation last summer, if you submitted an LIT application and/or reference forms in 2020, we have them on file and will use them for your 2021 application. However, you DO need to submit a portion of this application in order to activate your application for the 2021 season. Click on the appropriate application for instructions.)
- Send out 2 reference links (pastor/youth pastor and teacher/general)
